Special Dietary Accommodations
Overview
Hanover School District 28 provides meal accommodations to ensure all students can safely participate in our school meal programs. We work collaboratively with families using a team approach to meet each child's dietary needs.
Students with Disabilities
We will provide meal modifications when supported by proper medical documentation.
Required Documentation:
- Written medical statement or completed CDE form
- Must be signed by a licensed physician, physician assistant, or nurse practitioner
The medical statement must include:
- Description of the child's disability
- How the condition restricts the child's diet
- Specific foods to be omitted
- Required food substitutions
- Diet prescription or accommodation plan
Important Notes:
- Medical statements don't need annual updates unless the child's needs change
- Disabilities include conditions affecting immune, digestive, neurological, and bowel functions
- Each case is evaluated individually
- These meals are eligible for reimbursement even if they don't meet standard USDA meal patterns
Students with Food Preferences (Non-Disability)
We will attempt to accommodate special dietary preferences such as religious, moral, or personal choices, but these are handled differently:
- Medical documentation is NOT required
- Meals must meet standard USDA meal pattern requirements for reimbursement
- Evaluated case-by-case
How to Request Accommodations
- Download the Meal Modification form
- Have your child's doctor complete and sign it
- Return to the Nutrition Department at Hanover School District 28 or your school office
To discontinue accommodations: Contact our Nutrition Director for the appropriate form.
