General student registration for all families.

July 23, 2026

1:00 PM – 6:00 PM

General registration + Athletic registration & sports physicals (minimal fee).

Review all forms below before registration day. Forms marked with a doctor icon require completion by your child's healthcare provider in advance.

Family Economic Data Survey (FEDS) — 2026-2027

Determines eligibility for fee waivers and state/federal program benefits. Complete one form per household. Report all household income or provide a SNAP, TANF, or FDPIR case number. Also available online at secure.ezmealapp.com.

Transportation Registration 2026-2027

Required for any student riding a district bus. Includes the bus conduct contract, student release instructions, and emergency contact info. No student may ride without a signed, dated contract on file before the first day of school.

Immunization Records

Colorado requires all students to be current on vaccines: DTaP/DTP, Polio (IPV), MMR, Hib, Hepatitis B, Varicella (Chickenpox), and PCV13. Bring your child's up-to-date immunization record. If vaccines are missing, an in-process plan must be on file within 14 days of notification. Medical or non-medical exemptions are accepted.

Colorado Asthma Care Plan

Required if your child has asthma or reactive airways. Both a parent/guardian signature AND a completed healthcare provider section are needed. Specifies inhaler type, triggers, and green/yellow/red zone response instructions for staff. Valid for 12 months.

Student Housing Questionnaire

Helps identify students who may qualify for support under the McKinney-Vento Act (services for children experiencing housing instability). Only needs to be completed if your family does not own or rent your current home.

Out-of-District Choice Permit Application

Required for students who live outside Hanover SD 28 boundaries. Valid for one school year only — must be renewed each year by May 15th. The principal will notify families of approval or denial by June 1. Transportation is the parent/guardian's responsibility.
